At the Indiana Sleep Center all aspects of your care are administered through the combined efforts of our highly trained and credentialed technologist and physician staff. Our physician staff is also available to you and your physician for even more in depth consultation regarding your sleep health. If your physician believes you may have a sleep problem, they may either choose to arrange a sleep medicine consultation with one of our physicians or refer you to the Indiana Sleep Center for diagnostic evaluation. For further evaluation contact your health care provider or the Center for Respiratory & Sleep Medicine (CRSM) at 317-885-2334.

Physicians When referring a new patient, please download and complete the New Patient Packet below. New Patient Packet – download pdf New Patients Once a patient is referred to the Indiana Sleep Center there are a series of documents in a “sleep packet” that the practice hands out to patients. If you have a physician referral you may download our PDF files and fill them out prior to your sleep study. Insurance
Most insurance plans cover sleep testing, as long as it is medically necessary and ordered by a physician. The Indiana Sleep Center accepts most major medical insurance. However, every insurance policy differs from the next, therefore we encourage patients to review their individual policies and determine if their personal plans provide coverage for outpatient sleep testing procedures.
